Zeposia contains the active ingredient ozanimod, a selective sphingosine 1-phosphate (S1P) receptor modulator used to treat adults with relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease, as well as adults with moderately to severely active ulcerative colitis (UC).

Ozanimod works by selectively binding to S1P receptors on lymphocytes, preventing certain immune cells from leaving lymph nodes and migrating to areas of inflammation. This mechanism helps reduce inflammatory activity in the central nervous system and gastrointestinal tract, thereby decreasing disease activity and helping to manage symptoms.

Zeposia is administered as an oral capsule and treatment begins with a 7-day dose-escalation regimen to reduce the risk of heart rate effects, followed by a maintenance dose of 0.92 mg once daily. The medication can be taken with or without food and offers a convenient oral alternative to injectable therapies for eligible patients.

Clinical studies have shown that Zeposia can reduce relapse rates and disease activity in multiple sclerosis and help induce and maintain remission in ulcerative colitis. Regular monitoring may be required before and during treatment, including assessment of liver function, infection risk, and cardiovascular status.

Common side effects may include upper respiratory tract infections, headache, elevated liver enzymes, dizziness, back pain, and decreased heart rate during treatment initiation. Serious risks include infections, liver injury, vision changes, and cardiovascular effects. Patients should consult their healthcare provider regarding appropriate screening and monitoring requirements.
